InstaMed, a wholly owned subsidiary of JPMorgan Chase Bank, N.A. (“JPMorgan”), contracts with healthcare providers to offer services that involve processing medical, health insurance and payment information. This information is protected under the Health Insurance Portability and Accountability Act (“HIPAA”) and other federal and state laws. As a HIPAA business associate of your healthcare provider(s), InstaMed may use and disclose your protected health information only as permitted by HIPAA, InstaMed’s agreement with your provider and your provider’s privacy policy. In some instances, this includes sharing protected health information with other parts of JPMorgan to process payments, comply with banking laws and for other permitted purposes. In addition, if you are a California resident, you may have rights under the California Consumer Privacy Act as described in JPMorgan’s CCPA disclosure. Please note that JPMorgan’s policy covers a variety of products and services delivered to clients around the world. Consequently, some elements of the policy are not relevant to InstaMed’s business activities. JPMorgan and InstaMed do not use your protected health information to market products and services to you.
